Air. A cartridge change traps it, and a gun full of air compresses instead of delivering. A bleed nipple lets it out; without one you are pumping and loosening the head repeatedly.
Refilling from bulk grease using a filler pump, which is cheaper per kilo than cartridges if you use a lot. For a household, the cartridge is simpler.
